I, Parent/Guardian, for the Player and me, hereby release the Host Facilities, the Junior Players' Tour, LLP., its Officers, Directors, and Employees from any and all liability for any event or consequence whatsoever in any way arising out of bad faith or willful misconduct. Parent/Guardian and Participant, grants and assigns his/her individual media/photo rights respecting participation in the competition , without exception to the sponsors, PGA of America and the Junior Players' Tour, LLP. In the case of an emergency during a Junior Players' Tour, LLP event, I authorize a qualified medical physician to take the necessary measures in the treatment of this Junior Players' Tour, LLP participant. It is understood that neither the Host Facilities nor the Junior Players' Tour, LLP are responsible for any lost or stolen items. Signatures verify that both the Parent(s)/Guardian and Participant have read and understood the Association Rules as described. All Entries subject to field not being full. Should field be full you will be notified and credited. as stated below under Terms. Entries accepted by date received. TERMS: 1. NO CREDIT OR REFUNDS FOR WITHDRAWING 7 DAYS OR LESS PRIOR TO EVENT OR NO SHOWS. 2. $25.00 Service charge for withdrawing 8 to 14 days prior to tournament. No Refunds. *Applicant may apply credit remaining less $25.00 fee to a future tournament. *Must not exceed more than 90 days, after 90 days all credits will be forfeited with no refunds
